Schedule: Monday – Friday | 8:00 AM – 4:00 PM
Pay Rate: $16/hour
Pyramid Healthcare at York is dedicated to providing comprehensive, client-centered treatment for adults struggling with Substance Use Disorders. Our facility is a recognized leader in addiction treatment in York County and Pennsylvania, supported by a passionate leadership team and committed staff.
We are currently seeking a Case Coordinator to play a key role in coordinating aftercare appointments, maintaining communication with referral sources, and keeping legal or external agencies informed of client progress.
Key Responsibilities:
- Ensure clients have access to the full continuum of care by assessing needs and facilitating appropriate services.
- Coordinate all appointments, aftercare plans, schedules, and meetings.
- Arrange Halfway House interviews and placements.
- Obtain necessary information and releases from clients, families, referral sources, and other agencies.
- Perform other duties as assigned.
Qualifications:
- High school diploma or equivalent required
